Sports Program at Wingate Elementary School

Wingate Elementary has an exciting sports program in which every interested Bearcub athlete will have the opportunity to participate. Before any student can join a sports team, they must complete the Sports Participation Packet.

This packet includes:

1. Emergency Information Form
2. Medical History Form
3. Physical Examination Form
4. Consent to Treat Form

Completed forms must be submitted to the school prior to student participation.
